Website and booking terms

2. Website services

This website provides free information about the doctor, treatments, prices, appointment preparation and voucher designs. No account is required. Browsing begins when you open the website and ends when you leave.

General information does not replace a diagnosis, personal medical advice or an assessment for treatment. Treatment scope, contraindications and possible adverse effects are discussed during a consultation. Illustrative photographs do not document treatment outcomes. The separate Results section shows before-and-after comparisons; outcomes vary and photographs do not guarantee an identical result.

3. Appointments and vouchers

The contact form sends a request for contact. First name, last name, telephone, location and acknowledgement of the privacy notice are required. Email and message are optional. The practice receives the request and, if an email was provided, a copy is sent to that address. A request does not reserve or confirm an appointment. Do not include health information.

Voucher designs are previews. Online payments, automatic personalised PDF generation and delivery are not active. Selecting a design is not a purchase. Please call to ask about voucher availability and conditions. Separate purchase, payment and redemption terms will be provided before online sales begin.

4. Technical requirements and acceptable use

You need an internet connection and a current browser. JavaScript supports interactive features, including dialogs and the question search. Your internet provider may charge according to your agreement.

Do not submit unlawful content, attempt unauthorised access or interfere with the website. Do not send medical records or health information through channels that have not been agreed for that purpose.

5. Problems and complaints

Website problems and complaints may be reported by telephone, by email at gabinet@drosikowicz.pl or by post to the correspondence address above. Please describe the issue, when it occurred and how you would like to receive a response. Health information is not required.

A response will be provided within 14 days of receipt, subject to the legal requirements applicable to the matter. These terms do not restrict statutory consumer or patient rights.

6. Privacy and external content

Privacy and cookie information is available in the footer. The form sends data to the practice to handle your enquiry. The website does not run analytics or advertising, or save cookies or form data in browser storage. The hosting provider may process technical data needed to serve the website. Hosting and email are provided by cal.pl / INFO-CAL. See the privacy policy in the footer for details.

Maps and social media links lead to external services with their own terms. Website materials are subject to copyright law, including applicable permitted use exceptions.

7. Access and updates

You may read and save these terms free of charge using your browser. This version covers the informational website, contact form and booking deposit terms. New features, particularly forms or payments, require updated documents before activation. Updates do not retrospectively alter existing agreements.

8. Appointment booking and deposit (zadatek)

A PLN 100 booking deposit applies in both Sanok (BOTOX DAY) and Chorzów. First agree a specific appointment date and location with the practice. Before payment, you will receive the amount, deadline, recipient details and deposit terms. The contact form is an enquiry, not a treatment agreement or appointment reservation, and requires no payment. Do not pay before the date and payment details have been agreed.

Deposit terms must be agreed and provided before the agreement and payment. The appointment is confirmed after agreement and receipt of the deposit by the correct recipient. The payment deadline is agreed individually and communicated before payment.

Credit towards the treatment price

If you attend and the treatment is performed, the PLN 100 deposit is deducted from the treatment price. Only the remaining balance is payable; the deposit is not an additional fee.

Failure to attend and deposit settlement

If you fail to attend the agreed appointment for reasons for which you are responsible, the other party may withdraw from the agreement without setting an additional deadline and retain the deposit under Article 394 § 1 of the Polish Civil Code.

If the party providing the service is responsible for non-performance, you may withdraw and request twice the deposit. If the agreement is terminated by agreement, or non-performance results from circumstances attributable to neither or both parties, the deposit is refunded without double payment, under Article 394 § 3.

If the doctor identifies contraindications and the treatment cannot be performed, this medical decision does not automatically result in forfeiture. The deposit is refunded. Any separate consultation charge may only be settled on terms and at a price agreed before the appointment.

Cancelling or changing an appointment

Please notify the practice as soon as possible by calling +48 666 98 58 28 or emailing gabinet@drosikowicz.pl. A new date and settlement of the deposit require mutual agreement, subject to the rules above and statutory rights. Requesting a change does not automatically confirm a new date. Mandatory patient and consumer rights remain unaffected.

Use the payment details for your agreed location. In the transfer reference include the patient’s first and last name, appointment date and location. The BLIK number is for Sanok deposit payments; the practice contact number is +48 666 98 58 28. The deposit recipient and service provider are confirmed when the appointment is agreed.

These rules apply to new bookings agreed after publication and do not retrospectively change existing agreements.
